  4.             SUMMARY OF STATE LIVING WILL LEGISLATION

  8. Synopsis: Louisiana has a weak living will statute. The statute
  9. provides for oral as well as written living wills. If a person
  10. makes known to his physician the desire for a death with
  11. dignity, the oral declaration is to be made a part of the 
  12. patient's records, and is to have the same effect of a written
  13. living will. A minor's parents or spouse (if the minor's
  14. spouse has reached the age of majority) may make a living will
  15. for the minor, but must be unanimous in their desire for a
  16. living will. The living will does not have to be honored by
  17. a physician. To prepare a living will using the State of
  18. Louisiana recommended form, please select living will preparation
  19. from the opening menu. Remember that the publisher recommends 
  20. preparing a "uniform" living will in addition to the form 
  21. suggested with by the State of Louisiana. Please also consider 
  22. executing a living will for any State that you spend more than a 
  23. few weeks per year, or where you are a temporary resident.

  38. of revocation, by physical destruction (tearing, defacing,
  39. burning, etc.) It may be verbally revoked if the verbal
  40. revocation is communicated to the attending physician.
